ABSTRACT Acute alcohol intoxication (AAI) impairs the hemodynamic counteregulatory response to trauma and hemorrhagic shock (HS), blunts the pressor response to fluid resuscitation (FR), suppresses the HS·induced neuroendocrine response, impairs pro-inflammatory cytokine expression and increases mortality from infection during recovery. Studies conducted during this funding period examined a) whether the attenuated neuroendocrine response, particularly reduced sympathetic nervous system (SNS) activation, is the principal mechanism responsible for the hemodynamic instability seen in AAI+ HS and b) what the impact of AAI was on the integrity of host defense mechanisms during the immediate and delayed recovery from HS. We determined whether SNS activation can be restored by central (intracerebroventricular; ICV) neostigmine administration and whether this in turn is capable of improving the hemodynamic counteregulatory response to HS in AAI. Our results show that ICV neostigmine stimulates SNS activation and improves the recovery of blood pressure following hemorrhagic shock. Furthermore, our results indicate that this in part mediated by arginine vasopressin. Interestingly while the presser response to phenylephrine in vitro appears to be blunted by alcohol, the in vivo response to a presser with a different mechanism of action appears to be preserved. 15. Substance abuse as a mediating factor in outcome from traumatic brain injury.
A review of recent research addressed two questions: how common are problems of substance abuse in traumatic brain injury (TBI), and to what extent does alcohol and other drug use mediate outcome? Studies showed alcohol intoxication present in one third to one half of hospitalizations; data for other drug intoxication were not available. Nearly two thirds of rehabilitation patients may have a h...
متن کاملEffects of day-of-injury alcohol intoxication on neuropsychological outcome in the acute recovery period following traumatic brain injury.
Some researchers have found that day-of-injury alcohol intoxication is associated with worse outcome following traumatic brain injury (TBI). The purpose of this study is to examine the effects of day-of-injury intoxication on the acute neuropsychological outcome from TBI. Participants were 36 patients with TBI (18 sober, 18 intoxicated) matched on injury severity characteristics and demographic...

متن کاملBinge Ethanol Prior to Traumatic Brain Injury Worsens Sensorimotor Functional Recovery in Rats
A significant number of patients suffering from traumatic brain injury (TBI) have a high blood alcohol level at the time of injury. Furthermore, drinking alcohol in a binge-like pattern is now recognized as a national problem, leading to a greater likelihood of being injured. Our objective was to determine the consequences of a binge paradigm of alcohol intoxication at the time of TBI on long-t...
متن کاملA prospective study of the influence of acute alcohol intoxication versus chronic alcohol consumption on outcome following traumatic brain injury.
The purpose of the study was to disentangle the relative contributions of day-of-injury alcohol intoxication and pre-injury alcohol misuse on outcome from TBI. Participants were 142 patients enrolled from a Level 1 Trauma Center (in Vancouver, Canada) following a traumatic brain injury (TBI; 43 uncomplicated mild TBI and 63 complicated mild-severe TBI) or orthopedic injury [36 trauma controls (...
